Over the last month I have hung 6lbs for 4*20 minute sets a day no problem.

In the last few days I have noticed it is causing much discomfort at this weight. Whereas I use to handle it no problem, suddenly it is like I cannot handle this, despite no change in routine, hanger position, etc.

What has happened? Have I hit fatigue now and why? Surely you hit fatigue when the weight is new? Has anyone experienced this? How should I handle it? Drop weights?

Well, I think it means that you are gaining. So don’t stop, maybe drop weight on last sets? Sometimes it takes several days to reach lig fatigue, then it’s easy to keep with less hanging. I think, when you reach fatigue, you are gaining.

In my opinion, it sounds like you hit fatigue. It doesn’t come on the instant you introduce weight to your penis - it takes time. Don’t try to force yourself through the pain, just reduce the weight till you are comfortable again and continue hanging.
