I know this isn’t the right forum but I’m hoping the community will forgive me based on my circumstances.

My penis is messed up. That’s where I’ll start. I got a yeast infection from my girlfriend and didn’t get it treated in time. Now, my penile skin is super tight around my glans and shaft. I’m obese and after the infection I have developed a bit of buried penis. I went months without being able to get an erection without pain. Thankfully I can get an erection without pain now but my penis is covered in stretch marks.

Here’s the bad part. Every time I have sex or masturbate my penis skin splits open. The skin is so tight around my shaft that when I get a full erection and the skin is pulled back, it bursts open. Lubrication doesn’t stop the skin from splitting open.

My local urologist doesn’t understand what is going on. I’ve been several times over the past year. He’s given me like 4 different creams to try and the last time I saw him he straight up said he doesn’t know what to do. I’ve tried stretching exercises. I’ve built jigs based on skin regeneration forums and I’ve purchased various tools to help me stretch my skin. My penis skin just continues to split open. After which, I have to wait a week or so for the skin to heal and I can start again.

I need help!! I can’t keep living like this. Honestly guys I just don’t know what to do. So I’m coming to the community. Can anyone recommend a plastic surgeon or a clinic that can help me? My googling has lead me to couple places but can someone refer me somewhere? Please, I’ll take anything. I’m desperate.

I would suggest going to a different urologist and/or a dermatologist.

The primary goal of PE should be to make your penis as healthy as possible in both form and function. If you do that, increased size will follow.

I am certainly CERTAINLY not a medical professional and I will first reiterate that you’ve got to find another urologist/dermatologist ASAP. There are many niches within the medical community and there is almost certainly someone out there who knows how to help you if you can figure out how to find them.

That being said, a few things come to mind.
First, it sounds like you may have developed some extreme scar tissue from the long unresolved infection. If you feel confident that that’s a major component of your situation you might look into gentle methods of scar tissue reduction, definitely starting with something very safe and gentle. Topical Vitamin E, and topical iodine both have shown to have some efficacy in reversing and healing scar tissue. There are doubtless more advanced options and if you google search something like ‘NCBI scar tissue regeneration topical’ you might come across something helpful that your particular dermatologist/urologist isn’t clued into (again, remember that these physicians typically deal with a relatively narrow range of common ailments, it doesn’t necessarily mean that there aren’t solutions for you out there).
Second, could it be a skin elasticity issue non-related to scar tissue? What kinds of links are there between infections, particularly yeast infections, and changes in skin elasticity? Perhaps the yeast infection could have weakened some sort of local immune system mechanisms in that portion of your skin tissue, maybe having made way for a fungal infection? In any case, if you can be patient with yourself and keep doing research on what you’re experiencing and seeing how it may connect to other ailments, I’m sure you’ll find something.

Have your doctors used the word "phimosis"? I did some googling and came across a few links:

This seems to be exactly your condition albeit not caused by circumcision (if you read carefully it does say infection is the culprit). The mention of "lichen sclerosis" makes me think that a secondary infection or immune problem is in the realm of possibilities.

Anyway it sounds to me (again, a complete layman, see a different doctor!) like a scar tissue problem, and fortunately dealing with scar tissue is something that many different branches of internal medicine is interested in because it’s a symptom of many conditions. DMSO is mentioned occasionally on this site as a scar tissue tool although PLEASE do ample research on using it safely and particularly it’s potential negative impacts on eyesight and eye health. DMSO needs to be handled with extreme caution since it penetrates the skin very readily and carries any dissolved substances into the body.

This may sound stupid at first but I am also a fat guy, after you take a shower are you drying your penis? Take a hair dryer after you shower and dry your tool. What do you have to lose to try.

Prolonged infection, among other things, causes the connective fibers of the skin to be disorganized and it changes the mechanical properties of the skin, weakening its ability to stretch without ripping.

It takes time for the skin to recover the organization of the connective fibers and if it’s significantly disarrayed from repetitive injury it may never recover due to the initiation of cyclic remodeling and inflammation. This is how chronic ulcers sometimes form, which often have to be surgically removed to restore skin integrity since the disarray of connective fibers prevents the body from healing it properly.

I’m not a medical doctor, but I would stop sex and masturbation for at least a week and then re-evaluate the skin. To get a clear picture of what’s going on, if it persists, you’d need a histologist to take a sample of the skin and examine it under a microscope. It’s possible you’re still infected with something, I would use povidone iodine on the skin twice a day for a few days. It’s also possible you have particles of something embedded in the skin from having sex with an open wound that is disrupting the healing process.

You do not want this to advance to chronic ulceration since you will then have to have the skin removed. I wouldn’t have the skin removed if I hadn’t had a biopsy verifying the cause first or I had a threatening infection.

Chronic stretching only makes this worse by fragmenting existing fibers and building up more randomly alligned scar tissue.

You should be checked for diabetes. If you have diabetes then this situation is much more serious and diabetes may be the underlying cause. Knowing is critical for making the correct choice of treatment.

No infection can cause his penis skin to rip open from getting an erection. It also is highly unlikely that what the OP describes as stretchmarks is actually stretchmarks. Id be worried about a possible penile cancer causing this, which can be caused by prolonged HPV infection. Definately seek medical help.

Skin ripping is one of the hallmarks of chronic yeast infections in obese and diabetic people. The skin rips under their armpits, between the legs, between their toes, between their fingers and at the edges of nails. If the infection has spread to the backs of the fingers or toes, the backs can rip or tear when the finger is bent. It’s usually described as “cracking” instead of ripping, however it’s fundamentally because the connective tissue has degraded to a point that it no longer has the elastic properties to deal with periodic stretching. I don’t see why it couldn’t happen on the penis where the skin has to expand and contract regularly.

The OP is already in contact with a doctor, who hopefully would have identified melanoma readily if OP had it, though he probably hasn’t disclosed the stretching practices he’s been doing that could be aggravating and prolonging the process of healing.

Normally the lack of skin integrity wouldn’t persist long after the infection has been stopped.

There’s an unusually high variability in wound healing in humans though, so it’s possible the healing is stunted in some way simply due to genetics. It could also be diabetes that changes how wounds heal. It could even be due to the creams he’s been using. There are many possibilities to check. A histologist could sort this out pretty fast, but at the cost of cutting out a sample and probably an amount of money.

If it’s a diabetic microvascular insufficiency causing this problem, there’s not a whole lot of treatment that has been tested for safety and efficacy unfortunately. There are, however, a few substances that have promise in animal experiments that have a low anticipated risk profile.
