Due to the nature of the extender device (assuming we’re thinking of the same device), there is only a small area where the direction of the bend would differ, if I’m looking at this right. And that area is the base, which would be bent at a different angle.
Bending up would block the dorsal veins, causing the internal pressure to rise, but also reducing blood flow. It could also impact the dorsal nerves.
I’ve never observed gains from an extender (the next few days I observe a shortening of my erect length), but I always end up putting it on the highest tension possible.
