When jelqing, I find that I can apply most of the pressure either to the left and right sides of the penis (as when I use an overhand OK grip), or, to the top and bottom (as when I use an regular OK grip).

Does growth tend to occur in the areas where most direct pressure is applied, or to the areas that bulge out in response to the pressure applied elsewhere?

Growth occurs as a result of blood exerting pressure in the CCs and the CS. If you are using a overhand grip, it is fair to assume that the pressure is well distributed i.e throughout the cross section of the penis. If you dont have a overhand OK grip and if you are exerting pressure in the left & right sides then the CC will react more.

The CS sits underneath the CCs and as such wont experience much pressure.
