Kegels aren’t required, but they can only help ;)
They can help with some manual exercises too, like when you need a certain erection level for jelqs or in order to get full engorgement when doing manual squeezes and such.
Stretching downward focuses the stress on the suspensory ligaments. Over time, it lengthens them, which can be either desirable (some people think it makes their penis appear longer), but it can also have negative consequences such as a lower erection angle, and in extreme cases instability of the base of the penis, like when those ligaments are totally severed as part of penis lengthening surgery.
From my personal experience, stretching downward resulted in my erection angle going from 10-11 o’clock to 7-8 o’clock, which I kinda regret.
It’s better to stretch in as many directions as possible.
