First post here at Thunder’s. Long time lurker.. I am a healthy 25 year old male in good shape - could lose a few pounds, but in good shape.
This is a long post but I am trying to give you guys the most detail possible (and in the easiest way to read and understand). I’m going to try and take you guys through everything that has happened up until this point because this is something that is really bothering me and am genuinely looking for good advice. I know you guys know your stuff, so I want to hear your thoughts and implement your advice to solve this issue.

I’ve been at PE for about 5 or 6 years now through air pumping, extending, and jelqing with good results. I have seen about a half an inch in both length and girth for gains. Had I been more diligent with my routines, I’d probably be bigger but truth be told, I (was) happy with my penis. Through this time I do believe I did a small amount of “damage/tightness” to my pelvic floor, but nothing to cause any real harm or noticeable effects.

Ah, where to start..

Let’s start with an infection I had in my urethra back in May ‘19.

One morning I woke up in my (now ex) GF’s bed and noticed I had peed the bed. Long story short, I had green discharge coming out of my penis shortly after accompanied by something along the lines of hard flaccid and a tight-ish ballsack. I was tested for STDs but all came back negative (so did the girlfriend). The doc gave me that big white pill in case it was gonorrhea or chlamydia on top of antibiotics and everything went back to normal. I suspect I contracted that infection through an anti-inflammatory drug I was taking at the time for my hand (figured I’d include that). That, or when I had anal sex with a girlfriend of mine. Either way, it was just a bacterial thing and no trace of an STD was present. I did experience some hard flaccid and general tightness when I was infected with this, too (but all good after medication).

Then April ‘19 happened when I bought my first Bathmate X30. I had a healthy, low hanging penis up until I over-pumped when I first used my Bathmate X30. I am not knocking the Bathmate by any means, only stating I simply over-pumped and my penis turtled for the first time that day when I was on my bike. This was the first time my penis ever actually turtled - it really freaked me out. There was never any bruising or discomfort in or after using the Bathmate - my penis simply turtled that day and hard flaccid came shortly after. It has not been the same since that day.

It’s now about a year later, and my balls still do not hang low like they used to, and I have a semi-serious case of hard flaccid here and there. I’ve even wet my bed 6 or 7 times since using the Bathmate (granted, this happened over the course of a few months and actually hasn’t happened in a few months (and I was somewhat drunk each time it happened)), but it still never used to happen before. I have done reverse kegels, hindi squats, yoga, breathing exercises - you name it - to no real avail. There were times prior to the Bathmate where I’d hold my balls in my hand because they were so loose/low - they literally felt like they were 5lbs. In addition to that, I always had a good-looking flaccid penis when my balls were like that.

Now, everything is just tight and no matter what I do, I can’t seem to get it to go back to “normal”.

I went to the urologist for the second time today and I explained to him that I sometimes get a quick, shooting pain near my rectum that goes down between my legs when I’m reverse kegeling for a few seconds (usually towards the end of peeing). That, and when I ejaculate, I get a sort of dull/ache-like pain that does hurt in my groin, between my legs, and near my rectum. He prescribed me a 3-month supply of Prostate PQ (please do share your opinions on this). He said it will help me and it is the biggest thing that will/should help me. He does not suspect infection or hemorrhoids, but said he does think a tight pelvic floor has something to do with it.

I think it could be anything from a bacterial infection (though the urologist does not think so), to a simple scar tissue build up in the Tunica from over-pumping that is now causing my slight loss in girth, and slight loss in length. I also suspect that my groin is tight from actually pushing the Bathmate into my groin (you have to do that to create suction with the Bathmate unless you buy the Xtreme).

Other side notes to take into consideration:
-I still get morning wood on random occasions.
-I lose an erection very quickly after I ejaculate.
-My pee stream is also not as powerful.
-When I do push when peeing, I feel the pain in my rectum and it sort of extends to my glans.

Any advice/thoughts/experience is greatly appreciated.. More or less hoping someone has gone through this and knows what to do so I can get my low-hanging balls, and good-looking flaccid/normal-sized erection back. And of course, to get rid of the pain when ejaculating.

Doesn’t sound to me like you’ve got any sort of current infection. It does sound like you’ve got some sort of muscle strain/pull/sprain/tension. I’d do what the doc says and stay away from any sort of PE. Truthfully, I don’t think you should have messed with it at such a young age. Males keep growing into their early 20s.

I’d also like to add one more piece to this that I forgot to mention..

After I took the antibiotics for the infection, I was out with a few of my buddies a week or so after and ended up receiving oral sex from a random, very out of shape, and - pardon me - gross looking girl. I did it for a bet, and she gave me head. I live in the city and I can assure you she did not look like she was from a good area around here. Not that STDs care how someone looks, but without saying something that could get me in trouble, she very easily could’ve had an infectious/gross mouth. Very stupid, I know. I remember calling my doc the next day feeling like such an idiot and he was just like “if it was just oral, you should be fine.” So I went in and got tested ~24 hours after it happened. Obviously looking back I realize that was essentially pointless. But, I wonder if I contracted chlamydia/gonorrhea and it has made its way to my prostate. The doc never put me on an antibiotic because the tests came back negative (but also think I went and got tested too soon, no?).

Anyways, at the urologist today I did mention this and he is going to test me for chlamydia/gono (I did leave a urine sample today).

I’m thinking of going back and getting tested entirely (blood work, whole nine yards) for STDs.

I also noticed tonight that my pee is a little foamy, and so is my semen - almost looks a tad sudsy in a way.

You’ve given us very little info on your PE expect that you used the bathmate once. So unless your doing an extreme PE routine it sounds like it has zero to do with PE. You’ve got something going on that we can’t help you with because again this has nothing to do with using the BM once or even a dozen times. It sounds much more like your rolling the dice sexually. The STD test may be wrong as you have to factor in time frame.

Sounds like you’re on the right track. I will say that your doc is wrong on the oral standpoint though, you can very easily get HPV or HSV from that.unfortunately, I know. My wife doesn’t have Herpes Simplex Virus Type 2 (HSV-2), which is what you usually get through normal PIV sex. However, she does get occasional cold sores (HSV-1, common in 80% of people from normal, “wholesome” activities). After about a year together, sure enough, tons of pain peeing, lumps and everything on the shaft of my penis. I went to Planned Parenthood (figure they’re more accustomed to these sorts of things that my suburban primary care doc), they diagnosed it as HSV, but I have them test it and it was HSV-1. She was always careful not to give me oral if she had an active lesion, but the virus can still be shed in saliva.. So that’s how I got it. Unfortunately, she also gave me HPV, but I had them biopsy those and fortunately it’s type 6, which really doesn’t cause cancer.. And you can get that with a condom or not. So in any case, yes, you get catch stuff through oral.

If you’re talking about a bicycle, I would suspect that to be causing some problems, though not the infection.

It sounds like you have this.

When I was a kid I had to ride my bike to and from school about an hour each way with a gigantic backpack full of books the school required me to bring every day. My crotch would go numb while riding and due to the injuries I got my glans and CS didn’t expand during erection through my teenage years and early adult life. My glans is working much better now, though still not ideally, due to DMSO and regular penis pumping. I can get it to 100% inflation while edging now, and it’s been getting better and better.

I don’t recommend doing anal without a condom at all. Search for "poop noodles" if you don’t understand why it can cause urinary tract infections in men. If you feel poop up there, it’s already too late to put on a condom, so just always use a condom for anal.

I spoke to my doctor yesterday as well, and he too does not think there is an infection. So, I am not infected - think that was just me hyper analyzing the situation. Had I gone full-blown sexual intercourse with that girl, it might’ve been a different story.

I think a lot of it has to do with the bicycle riding as well. My doctor also thinks I have a tight pelvic floor.

I read something on PE Gym about a guy who fixed his posture sitting down/standing up because apparently the pelvic floor is in a bad spot when hunched over. Not sure how that works, but that’s what I read. If you think about it, you’re hunched over on a bike.. Makes sense to me. Combined with the pressure of the seat, etc., I think using the bathmate on top of riding the bike stretched and already tight pelvic floor and my penis reacted by turtling.

That said, my urologist has me taking Prostate PQ 2x a day to help with - what he suspects - an acute case of an inflamed prostate (from having the urinary tract/pelvic floor agitated).

Side note: I think my underside of the pelvic floor is OK/somewhat loose and flexible. I think the issue lies in my ligaments above my penis in the pelvic region. I also am going to lose a bit of fat there to see if I can’t stretch them out/get better blood flow there.

Pretty wild how all of this sort of comes together - definitely took me a while.

Men shouldn’t ride bicycles. Maybe they could be made safe with some adjustments to the seat, but for now they should be carrying a warning label.

If surgery is recommended, I would take a few months to consider the risks and see if it resolves itself after stopping bicycling. The risks to pelvic floor surgery are not something to take lightly, especially if the goal is decompression. Personally, I wouldn’t do it. Consider the sense in decompression surgery of a nerve, the nerve is surrounded by tough connective tissue that is entrapping it and the goal of the surgery is to cut the connective tissue along the length of the nerve if I remember correctly. In addition, the normal surgeon is still using blades for these kinds of surgeries instead of enzymatic blades because Western medicine is strangled by private interests that can’t create a captive market from enzymatic blades. You really have to try to cut a nerve with an enzymatic blade, but with a glass or metal blade nerves are like butter. An enzyme blade is basically two sheets of very thin plastic, collagenase or whatever enzyme cocktail you need flows along the outside of the blade then gets sucked into the space between the sheets. The tissue being operated on is slightly submerged in saline so the enzyme only comes in contact with the tissue you’re cutting. Enzymatic stitching isn’t perfect yet, but if you’re doing a decompression surgery you’re going to be using traditional sutures anyway and that’s worse, at least you won’t cut a nerve with enzymes. The only downside with enzymatic blades is the loss of tissue, which can be as low as a millimeter and the potential for mechanical or surgical failure laminar flow fails and enzyme degrades the surrounding tissue.

I think DMSO is worth considering as well after you stop riding bicycles. It’s helped me significantly.

The way that typical repetitive bicycle injury works in the perineal region, as I understand it, is that the compression from the seat causes ischemia, when you get off the seat it causes reperfusion injury. When tissue is under oxygenated mitochondria make adjustments in how they use oxygen, when blood flow is restored suddenly the increase in oxygen causes mitochondria to generate a lethal amount of free radicals that initiates apoptosis, the apoptosis sets off an inflammatory cascade for the neighboring cells. Either the lack of oxygen itself or the inflammatory cascade causes fibroblasts to collagenize tissue. Other things go on too, but this is the relevant stuff. If this is done repetitively, the connective tissue can become disorganized, causing cyclic remodeling and inflammation.

The perineal region is not meant to be weight bearing and the anatomy of the area is among the most delicate in the body, which is why bicycle seats cause so many problems.
