A lot of people find it so. I think there is more potential for nerve damage than with manual jelqing. As with manual jelqing you should be aware of the location of the nerve close to the surface running along on the top of the penis and take care to avoid the nerve bundle just before the glans.
Some people have also successfully used the device in a vertical orientation and logged good results (especially for girth) but the chance of harming the nerves could even be higher than using it in the standard manner if rotation of the shaft occurs near the end of the stroke.
Many people use this device successfully and without problem, if you are careful you will too. A fairly common comment is that the device creates a good grip on the internal penis and thus produces a good stretch.
Having used it myself, I initially thought it was worthwhile but have found that manual jelqing is a much better method. Of course it does take some time to learn to jelq well.
