Any experience with "screw" type extenders?

Certainly seems to be the case for me too, but I have read a lot about this in the past 2 weeks and some people say that for them the glans don’t get cold. And that even 20 minutes of cold glans can cause nerve damage. Other comments say that nerve damage only happens when the glans turn blue/dark purple.
I have used my extender for 9 hours the past week, for one hour or less for the first few days, and 2 hours for the last 2 days. My glans got cold each time, after around 15 minutes. I tried multiple ways of attaching, noose, strap, silicone tube with flat middle part, velcro and I bought a bunch of different pads and bandages to wrap my penis, but they all have the same results: they either slip off, or my glans get cold because they are tight. I would strap my penis in, and get it tight enough to not slip, then I would try to loosen it a little by little, but there was no sweet spot where it would both hold and not be too tight.
To be honest so far I didn’t experience any loss of sensation or side effects so I will probably stick to doing this. Hopefully any potential damage will be gradual so I will be able to stop and recover if I need to.
I am considering buying a VAC ADS and adding the VAC part to my extender, as I heard that’s the least dangerous way and it doesn’t restrict blood flow but I don’t really like the side effects: blisters, and fluid build up, and the fact that you have to put tape and other stuff on your penis.

I don’t know if this will work for you, but it’s worked great for me…take a look at my post here: Cunny Linguist - Success with the Quick Extender Pro

My approach was definitely against the advice of the majority on this forum, but I just didn’t have the time to go for moderate stretch over many hours and days. That said, there were a few posters on here who claimed to have made erect gains using my approach. Probably not much help to you, but that’s my limited experience of this type of extender.

I would like to see a pic of the modification to the screw type.

I have never tried an extender like this, but I have seen many improvements in this style of rod extenders since I started.

I started with an extender that used a single round silicone band and it was effective, but had many issues with pain behind the glands. I may at some time try a strap style extender again, but we will see. I look forward to seeing how this works for people over the years.

Modified Slider Extender

The only actual modification was to drill holes in the Vac Extender Head to allow the rods through. I added two washers between the screws and the Vac Extender Head to allow for a smoother extension.

I think this is very specific to the person, their size, etc. I so far have never had any fluid buildup in the head of my penis. I also have a glansxpands pro cylinder. I have drawn fluid in behind the glans, but can hardly notice any difference to the glans. I purchased a penimaster pro kit. I used the included glans chamber and ordered 2 larger sized chambers. The larger is much more comfortable. It still makes it sensitive, but not nearly as much. My head just barely fits inside the larger sized glans chamber. So I like that it comes out not looking like the Liberty bell. I may be the cause of the sensitivity. I have my extender adjusted so the springs are completely bottomed out. It could be the extra stretch is too much 🤷‍♂️
