I’m using monkeybar’s Modified VacExtender headpiece with my FastSize base/tension bars. It works pretty good, but I’m currently taking two days off due to some dark-red bruising right around the urethra. I’m hoping I’ll eventually get “conditioned” to the vacuum, so this won’t happen (I’ve been using it for about three weeks).
It seems like the grasping mechanism is the “problem area” of every extender. I’ve used both the noose and the “comfort strap” and neither work for me. The monkeybar headpiece I can wear for an hour to 90 minutes, but the head is very purple, “shiny” and a bit deformed looking (although not as much when I started).
Dongalong, is it your experience that you eventually become “conditioned” to the vacuum? I can see myself easily doing 8+ hours with the device, but not if it’s bruising my glans like it is currently.

I like the look of monkeybar’s latest solution, it’s a shame that it has been discontinued. It grips behind the glans so allows a stronger pull without the risk of vacuum damage.
A question to other users: Do you always set your extender to maximum tension or do you wear it after your PE session as a ADS type healing device (at lower tension)?
