1. Problem with the tension marks?
The tension marks don’t move so I don’t know how much tension there is. No matter how much I move the screws the tension marks don’t move so I don’t know how much tension there is. I am afraid that it restricts blood flow/Circulation and in the end I will loose my Penis instead of making it bigger. I am using the comfort strap because I think its better than the silicone tube. I also wrap my penis with a self adhesive bandage, I find that its the best for comfort. If it restricts blood flow what would be the best way to use it? Is it possible to use it without restricting blood flow? If it restricts blood flow after how many minutes/hours it becomes dangerous? Should I have breaks?
3. What is the best way to use it?
How many hours a day for first week, second week and so on? Breaks in-between sessions?

1) Extender tension is created by the springs pushing the upper section (sled; carriage) away from the base. When you strap your penis onto the sled, you need to compress the sled down to the base. Once strapped in, you release the upper sled section and the springs push it up and away from the base, placing the penis in tension. The springs should be in a compressed position; if the springs are uncompressed they are not providing an tension force to the penis.
Two things you can do if this is the case. First, you can unscrew the tension rods, which will compress the springs. Doing this should move the tension marks towards the upper rod. If this does not work, the second thing to do is to restrap your penis in, making sure the springs are fully compressed. You may need to reduce the length of the rods also.
2) Yes the comfort strap or noose restrict blood flow to the glans when strapped in. The noose does this more so than the comfort strap. You want the strap to be just tight enough to hold your penis in place. After using it some you’ll begin to develop a feel for how much is needed.
Some extenders have foam tubes to slide over your penis to help cushion the strapped area. Most any kind of wrap will help. It needs to be just tight enough to stay in place until you strap in. The strap will hold it in place after then.
In all cases you should monitor your glans for numbness, coldness and white spots. These are signs of loss of circulation. Try kegeling in fresh blood every so often. Also you can unstrap and massage your penis and then strap back in.
Total loss of blood circulation can cause tissue death in as little as 20 minutes. That level of strap tightness is not needed, and unless you are using a wrench or pliers it is unlikely you can tighten the strap that tight.
One hour is about the longest I could stay strapped in to my extender, after which I’d take it off, massage my dick and then start again.
It will probably take some time to get your penis conditioned so you could comfortably use it for an hour. Try 15min, then in a few days try 30 min and build up, etc.
