Well, usually you want to pull on the head to extend it fully from the front of the hanger. This allows the hanger to grasp the internal structures further toward the base and helps keep the front of the hanger away from the head.

If you wrap too tight, and put the hanger on with too much wrap in front of the hanger, when the hanger is fully tightened and weight is applied, the wrap becomes a constriction and hinders blood flow. Some guys have had problems with this and with fluid buildup in the skin below the head.

My fault. Some guys hang without wrap and do not have the above mentioned problems. Others who try without wrap get the skin caught in the teeth.

It will move down some, but you do not want blood flow consticted by the wrap. This defeats the properties of the hanger.
