First and second cylinder theory

Can somebody explain me the theory of use of two cylinder, first little and second more big, like in Dr Kaplan site?

According to Dr. K, the big scrotum and penis cylinder is for length because it pulls at the ligs from underneath and around the scrotum, and the smaller cylinder is for girth. I use both tubes but I’m not sure I agree with the Dr.s theory. To me it seems right the opposite would be true.

I see some gain in flaccid or erect lenght with my pump, it’s not a big scrotum, but a normally 13/4x9” pump.
