I think it is a pretty simply issue. If you end up with a donut, it means too high a pressure and/or too long a session. Most of the pump "pros" advocate slow and low pressure pumping to try to avoid the donut. It may still happen with length of time. But, if you try to use a graduated sizing (pack a thinner tube first, no where for donut to form) and even use a sleeve or smaller frenulum only sleeve, you may be able to restrict that area’s expansion.
But, bear in mind that the donut (Michelin Man look) is fluid atop your tunica below the skin, so its not really helping the long term tunica expansion, which is actual growth. I know if I had a productive pump by the feel as I do a few jelqs post pump. If hard or close to it, then is was productive. If you’re first squeezing a soft layer, then you’ve just filled the space between the tunica and skin with fluid.
