Seeing urologist tomorrow - need help with what to say

Hey all, seeing a urologist tomorrow morning at 8:30am. I’ve seen him once before, in 2019 when I overused a Bathmate X30. Turned out to be fine and all was well - he told me not to worry about it and he was right.

However I really feel like something is off this time around and I think it is infection/inflammation related. That or it’s something pelvic floor, or perhaps diet, related. That said I still want to rule out infection and want to ask for a urine test and semen analysis (how to do that?). This is a quality of life thing for me. I want to tell him my symptoms but there are a few… As follows:

- frequent urination and dribbling on myself after walking out of the bathroom.
- urine is sometimes cloudy and have taken at-home UTI tests and it has shown traces of leukocytes, as well as high-ish microalbumin which I read can be kidney related. Have gotten urine tested before and its been negative - however have had two new partners since then.
- low semen amount and sometimes yellow, even though I do masturbate occasionally.
- smaller than normal flaccid state, hourglass shaft and cold to the touch - also very tight feeling with scrotum also tight.
- sometimes sharp pain in pelvic area below belly button but rare, and sometimes sharp pain that feels like an intense spasm coming from the right side of my rectal canal (this makes passing stools uncomfortable and I get a “numb” feeling at the opening when passing stools). Sometimes the feeling comes back and it actually feels like my bowels are being emptied easier (sorry for gross detail).
- no daytime erections or urges to have sex. Morning erections very uncomfortable but they’re there sometimes.
- mild hydrocele in scrotum (was not there pre-bathmate).
- weed is really the only thing that can get me going - feels like it relaxes me.

In short it’s been 3 years since the BM injury and it probably doesn’t have anything to do with that, I’m just saying that I’m trying to get a diagnosis so I can at least work at getting better, the issue is that it’s sort of complex and totally admit that it could be mental, but there are certainly physical things going on that are very real for me. I also had a case of non gonococcal urethritis in 2019 that I think is wreaking some havoc on the pelvic area because it’s somehow in the prostate, bladder, or hiding out up by the kidneys. It’s just not the penis that I’m used to and for the past three or so years I’ve just sort of dealt with this. With the new girl and me getting older I’m starting to take this more seriously, you know? I just want to convey this in a way to the uro in an effective manner - one that will make him happy to help and not look at me as a dumb 28 year old guy.

Thoughts?

I had a similar issue when I was a kid. Some of the worst pain I’ve ever felt. Almost like you said, sharp pain sometimes making it painful to pass stools. Ended up being a prostate infection/inflammation.

If the tests are negative then it sounds like Pelvic Pain Syndrome.

Tell him your symptoms and allow him to be the doctor.
