It’s safe to say that your plumbing works. But nocturnal erections and actual intercourse are two different animals. When asleep, your parasympathetic nervous system is in command. That one encourages erections. While awake, your sympathetic system is in control, meaning that your conscious brain is also involved and various inhibitions can come into play. This opens the door to many orgres, like Performance Anxiety - a form of ED. Trick is, figuring out what the heck is going on with the individual when there are problems that tests seem to rule out.

Of course, mechanical problems such as venous leakage are more worrying, and as long as I’m in the clear in this respect then I’m a happy man. It makes sense that mental factors such as Performance Anxiety would come into play during actual intercourse, but these seem to be more a problem with state of mind, and thus much more easily overcome. It’s one of those mind-over-matter things, it seems to me, only the more you try to control the less control you have. It’s like an inverted mind-over-matter, but stil mind-over-matter. I used to have a much more serious mental problem with erections, including the stress-induced adrenaline release which you described above, but I’m much better now. I take this improvement as a sign that I am getting better and will probably continue to do so.
