Your right, mine does not have anything to “crank”, i saw an extender a few minutes ago that had “screwable” rods that could be adjusted once it is on.
Mine does not offer this, mine has extension rods and multiple sizes, but those screws on the bottom near the pelvic bone, are the only “screwable” items to tighten, but does not really give much tension addition.
My Extender only has one set of springs.
I agree with you, i wish i could “tighten” the tension every 30 min…even at my desk job, i can easily go to bathroom and adjust.
I have also read, more tension = quick results, but at the same time = easier injury, but there is a “middle” we need to find.
Im using the noose now, the comfort strap sucked for me, never gave a good pull and always slipped.
Yesterday i wore the noose with a bandage, now just raw and seems to be holding very well.

I definitely had some “skin” pulling rather then lig pulling with the comfort strap, the noose graps the head better and i feel it more in the ligs.
Do you use any formal for the initial setup?
Meaning, if you are 6.5 PBEL / 6.3PBSFL
Would u load the extender to 6.5 inches and use, or use 6.6 inches or even lower at 6?
Asking because i read most extenders are PBEL - 4cm

Here’s why I bring this up: My extender comes in 2 pieces, and extension bars. I place the base unit on first, and I strap on the 2nd piece separately. Once the top piece is securely fastened, I connect the 2 pieces together. After which, I slowly crank up the tension to the desired level.
I’ve been using an extender for 8 mo’s now. I started with 2-3 notches for 1-2 months, but have increased tension to 1 1/2 - 2 notches, never more than 2 notches. My extender came with 2 springs, and I’m using the higher tension spring. The tension should be dialed in, without pain, yet feel a nice firm pull. You shouldn’t be able to pull on the end of the extender further than what the tension is creating already, otherwise you don’t have the tension set high enough.
I have to adjust my tension about every 15 minutes, so if you’re wearing it under your clothes, you may not have this luxury. If you go to long without adjusting it, the tension may drop to an ineffective level. I’ve read that higher tensions can create quicker results. Hopefully that’s not just some theoretical marketing ploy, but still, it makes sense to me. Of course one should never set tension to point of any pain whatsoever.
I’ve read on TP that many extender users like the noose, especially after the skin toughens up. I prefer the comfort straps only because the noose causes pain at my desired tension, and I think it’s just personal preference.
The tension should cause the base unit to be firmly affixed to your pelvic bone. If not, you may need base unit with a wider opening. One problem I could envision especially with too small of a base unit opening is that the tension is pulling against your skin more than the internals of your penis. Maybe the extender can be used to modify from cut to uncut & desirable. HA HA!! Yet I digress. I actually place a cushion between the base unit & my pelvic bone because I used to develop a dull pain on my pelvic bone after a while. Since I’ve been using the cushion, I have no more pain.
