Few people actually apply "great care and vigilance" to anything, let alone PE. In particular, those just starting out with PE often don’t even understand what "great care and vigilance" means with respect to PE. Also, while the safety of others is generally the main concern, there are liability issues surrounding recommending the use of a product known to be painful/unsafe.

If you use the terms cheaper and expensive in a discussion like this, please at least provide examples of the costs that you consider cheap and those you consider expensive. The reason for this is that people all have different assumptions about what prices are cheap and expensive. In addition, for most people, the safe variety (e.g. AutoExt , AutoExtender Reviews, $139) is less expensive than the noose type (e.g. JES (andro-penis) from AndroMedical , 299€ = $352.51). Thus, without examples, we look at your statement and go :wtf: ?
You are obviously using different examples than the ones that I have used. That is fine, but we need to know what your examples are in order to provide comment on your statement.

If you just want an ADS, you could make one yourself for a very low cost.
However, my personal preference is the AutoADS for $59. I have been using it for closing in on two months. It is comfortable enough to wear for 12+ hours per day (my max is more than 24 hours, excluding bathroom breaks, etc.).
NOTE: I consider an extender to be different than an ADS. An extender is usually designed to provide a controlled tension on the penis for reasonably significant periods of time. An ADS is designed to keep the penis Stretched All Day (ADS= All Day Stretch). The amount of tension provided by an ADS varies greatly depending on type and actual use.

As to the noose type: I tried one (from a reseller of the andro-medical, JES). I found it very uncomfortable, and nearly impossible to wear for extended periods of time. I purchased the Modified Head that is much more comfortable. However, due to the better design of the AutoExt base (accommodates base girths > 5 5/16), I wish that I had replaced the entire unit.
Basically: You get more benefit out of an extender if you wear it for longer periods of time. If it hurts, you will not wear it for as long. Alternately, people will just stop using it.
