If you are doing manuals now and have not gained, I would suggest doing three things before you jump into an extender:
1. A-stretches (manually) hold for 1 minute, start with like 3 for 1 minute then work your way up to whatever you think your penis can handle
2. V-stretches (manually) hold for 1 minute, again start with like 3 for 1 minute then work your way up to whatever you think your penis can handle
3. O-squeezes: get your penis rock hard, then squeeze the base as hard as you can making a circle (your thumb and index finger should meet) hold this squeeze for 30 seconds in the beginning and work your way up
With manuals, I think I did 3 days on and 1 day off with a morning and night routine. After a month and after I measure, then I make adjustments to either how long I hold the squeeze/stretch or how many times I do the exercise in one routine. During the 3rd month, I always rotated a manual stretch, jelq, or squeeze each time I went to the bathroom and still did my morning and night routine.
I always progressively increased either time or the number of times I did an exercise so my penis wouldn’t immediately get used to the exercise or tension. If each month you are gaining either BPFSL or BPEL, then I would stick with the manual exercises. I would only change over to an extender after you stop gaining from manual exercises.
Also, you will then have an idea if the A- and V-stretches worked as well as the squeezes and these are the exercises you need to incorporate into whatever PE routine you do.
