The glans don’t tend to get as hard as the rest of an erection. I noticed a difference after I’d PE’d for a while. It wasn’t that my glas were any softer, simply that my erection was harder.
Have you checked your morning erections? Its quite possible to sleep through them, or wake slowly and miss them. Anyway to check that you are getting them a simple test like wrapping the penis with something too small for erect penis that will break when you get an erection will help confirm this. Some time a ago a guy suggested using stamps for this, making a ring around the penis. I’m sure a perforated self adhesive label for a printing would do the job as well.
Spotaneous erections have a habit of dissapearing if you are worried. Nothing kills an erection like worry.

This sounds terribly complicated. The fact that you can get an erection proves that you have no physical problem, losing an erection may well be a matter of worrying about losing your errections and if you can concentrate on what your glans are doing while you are ejaculating you are a better man than me.

If your urologist did a full exam of your penis and gave it a 100% OK, you are probably on fairly safe ground.
Its very easy to conflate the physical functions of the penis with the mental functions that control it. To me it sounds like the physical side is in all liklihood fine and that its the mental side thats the problem.

So you are jelqing now and trying to evaluate whether your erections are generally getting better of worse? Stop jelqing for a week or two and then try if you feel the need.
If you are experiencing pain of any kind go back to your urologist as Jersey says and tell him/her you are expereincing pain.
Other than that, PE is something that causes us to look at our own dicks with new eyes - the most minor tremble in an erection can become exagerated, the natural loss of an erection during jelqing an inability to keep one. Things you would not have noticed or thought about before are maybe topping the thoughts in your head.
This is a problem and it is a form of ED, and though the problem is not physical, it can be just as threatening and just as likely to get worse over time especially if you don’t recognise it for what it is.
