This is a rough guide I’d say. Your aim is to protect the fragile nerves just below the head. You should do what is most comfortable for you, as simply stretching skin is uncomfortable and futile unless you need it to cover an increased size. Personally, when stretching, the webbing between my thumb and forefinger would be just behind the head.

What’s your LOT? Once you fully understand the LOT theory it becomes obvious which direction to major in. I would cover all bases, especially when you’re starting out and do all the directions, but perhaps focussing on one more than the others. For example, it is typical to start with downward stretching to get any “easy” gains you can out of the ligs before attacking the tunica (stretched using the upper angles, above your LOT).
The key is to concentrate on how the stretches feel, as otherwise you will just be going through the motions. While this will work from some people, others require specific angles and intensity to elongate the necessary tissues.
As for duration, that is again a personal choice. I would typically do 30 second stretch in one direction, and then 30 seconds in another. Then I would do JAI stretches maybe (those are Johan’s AI stretches if you need to search) and maybe a rotary stretch lightly for a while.
I don’t think I can stress enough how important it is for you to pay close attention to how the stretch feels.

Now this is hardly standard advice but I’m ok with the blood in the head, it gives some engorgement and it gives something good to hold on to.
The warnings against this though:
1) If I don’t warmup and then stretch like that damage occurs, i.e. red spots which are harmless enough, but given the need for safety I would definately advise a warmup if your stretching sessions are like that.
2) You should periodically do some dry jelqs to get some fresh blood to the head if your stretching session is going to involve this level of engorgement to the head.
3) Having the head to hold on to gives you incredible power, don’t get carried away and injure yourself.

The break between your stretches depends on the length of the stretch. If you do 2 second stretches (JAI) then a 2 second break is in order. If you hold your stretches for a full minute, then perhaps 20 seconds of massage would be recommended.
For the second question, you should occasionally refresh your penis with fresh blood with some light jelqing.
Ideally, you want to be rid of most blood for stretching for safety’s sake. You don’t want an intense pressure pushing out laterally as you stretch. That would be asking for trouble.
