I have cheap LeLuv rod extender that I added springs to, a Best extender and a Total Man latest version. The Best and Total Man have c-shaped bases which are way more comfortable for someone girthy. Both are primarily designed for vacuum cup setups, though you could use a low profile compression hanger with them if that is your preference. Total Man sells compatible compression attachments, both as a ‘compression plate’ and as a ‘compression hanger.’ But I don’t prefer compression for anything, TBH.
I never use my cheap eBay LeLuv rod extender as the base is too narrow and too hard, the construction is flimsy and I don’t like the noose style. It’s not really made for high tension extending to replace hanging, but made for long duration, low intensity extending. It might work if that is your interest.
The Best is really great. I have the previous version, and the newest version is even better looking. It’s bulkier and less discreet than the Total Man so you can’t use it for ADS in stealth mode, but I like the built in calibrated spring tension so you have an idea of how much tension you are using and you can do progressive load over the session, like with hanging. You can also do bundled extending, since the catch hook rotates on the cross bar. This one is a bit pricier than most but it is a really high quality and worth the money if it is in your budget. I think the new BE Pro one has a built in digital scale and bluetooth connectivity for tracking your sessions if you are interested. I just checked the site and it is on pre-order for $100 off the planned price, so if you want to get one of these it is a good time to pre-order. Site says planed ship time is mid-September.
The Total Man is also really good. It’s much more affordable than the Best. The profile is a bit lower and it is lighter weight than the Best and there are videos of the owner putting it on under pants, so you could go low tension, long duration in stealth mode if that is your interest. It has these calibrated lines on the cross bar that are supposed to tell you how much tension is being used, but I found it doesn’t match my fish scale tension, so not necessarily useful except as a relative measure. But a nicely crafted low tech feature is that you can add these additional rubber bands across the cross bar to the catch hook to increase the tension at the same length, so like the Best you can add or subtract tension over your session if you like. There are also cross bars that snap on to fix the extender in a SO position and others to put along the extender to do fulcrum extending if you like.
So both the Best and TM have more features that could be useful.
Some sessions I use one, some the other, depending on what I feel like working with, but IMO either one is a good choice if you want to do high tension extending.
The Hog and Apex are other options, and though I don’t have them I think they are kind of the same design.
