Any compression hanger will create some non-permanent indentation at the sides of the shaft, right under the glans, because you need some place for the weight to rest on. These are called “shoulders of the head” and will probably go away after some months from stopping hang sessions.
About scarring along the shaft, If you mean skin striations because of skin elongation, that´s totally possible. It will stretch your skin too. If you mean scarring in the septum or outer tunica, I dont think that´s how it works… would be like wondering if an athlete has scarring along the tendons. It will strengthen your tissues though, as is normal if hanging weights.
It can cause other side effects like thrombosis in veins or lymph vessels, inflammation, etc.
