Is there much of a difference in using an extender vs hanging?

In short, yes.

The JES can apply app. 3 pounds worth of traction and is intended for extended use on a daily basis. For as long as you can. I own one, but due to numbness I can´t wear it for more than a couple of hours.

Hanging is, from what I have read and experienced, much more intense (and dangerous) due to the heavier weights for a shorter period of time. I have read about people hanging as much as 30 pounds and more. My solid believe is that these guys are circumsized and are putting most of the stress on their skin actually. Guys with unbelievable weights that is. Any uncut guy will probably second that.

Hanging is much more time consuming. It requires much more dedication but can give excellent results. Some extenders say that they can be worn around outside under your clothes and would give you in THEORY some ability to do other things at the same time. I seem to hear often that extenders are okay, but are not as easy on durability and people break them more often.

When you hang, you ALMOST always have to be at home and have some privacy.

I think uncut hanging is harder to master, but once you get itdone, it is basically the same with a wee bit of difference. Look for threads on hanging by the UK hanging guru SS4JELQ.

Is it not “dangerous” to looping silicone tube around the corona, like in the Jes?

It really is not the best idea, but there has been modifications people have made to replace noose like tie downs with something more ergonomoically friendly. That way you can use the device with the tension legs/stilts/arms and add the modification and you are all good to go.
