We are trying to heat the tissue. For making any material heat up we need to put energy into it for make it happen.
Diagnostic and most of the cosmetic use machinery operate at energy level not enough for the temperature rise we are after.
Therapeutic ultrasound, the intensity of 1.6 w/ cm^2 is the minimum. 1MHz or 3 MHz.
