You bring up some really good thought provoking questions here- and I hope a lot of guys chime in on this one.
As you can imagine, I think the thicker the penis is the harder it will be to elongate. This applies only when PE exercises have been done over a period of time to achieve girth, however. The reason I limit it to PE gained girth is because the scar tissue will be greater and it will be more difficult to lengthen. If a guy has a naturally thick penis and has done no PE to add girth, I think the length can be achieved if that is the focus.
IMHO growth occurs everywhere. The idea is, as Bigger has said, to divide and conquer. I do not think this applies only to the ligs- it applies to all the tissues. IOW, I think you have to focus the majority of your effort on one thing at a time to achieve the stresses necessary to achieve growth. When I am stretching skin, I focus on getting through that barrier so I can the focus on the ligs. This focus would apply to all PE techniques for optimal results. As the saying goes- ‘Jack of all trades, Master of none.’ Focusing on one growth aspect at a time should in theory at least promote optimal results.
You are correct that the Bib is placed 1” behind the glans for a good portion of hanging time. I personally move it around on the penis to prevent fatigue or overcompression of the tissues in the penis. However, we place the Bib there because it will generally slide a bit forward and rest just behind the glans- which provides the same type of stretching emphasis as you would get from manually grasping around the glans. Some of us do move the hanger all the way to the base for some of the sessions to focus on skin and tunica stretching.
