Start with putting the extender together in the length that you are able to put your penis in with the head almost in front of the extender ( or the cradle to be more presise). You will want the noose to pull downwards and not so much forward to keep your penis in place. This will be most comfortable.
So, put together the extender and push it in with one hand, mount your penis, and release it, so it extends and pull your penis as far out as your bpfsl go. Then you will notice that you have great tension, but if you want more, adjust the screws to desired tension.
As I said in the beginning, take it easy, 600-900 grams of tension. If you have much more tension than that when released the extender, you will have to start over again with a shorter extender. Stay with that tension for at least a month or two, to get your penis comfortable to the situation. Try to stay in the extender for an hour before you take a little break. In the first month or two, try to clock in 2-5 hours a day.
After that time you can adjust to higher tension, maybe do the hours at 900-1200 grams for another month, then you can go to 1500 grams. I think that almost all extenders have this grading.
Remember that every man is different, and there is some differences in what works for me, you and everybody else.
To use myself as an example, I started out with a flaccid that were shorter than my erected, but that has changed as my bpfsl now, is 1,3 cm( 1/2 inch) longer. You will see gains very soon because they come easy in the beginning. The first 1-2 cm is «easy», after that things go a little slower, but be patience, it takes hard work and commitment.
