Stretching down does target the ligaments more, and stretching up does target the bottom of the shaft more. But if you stretch to the right, you feel it in the left (and if to the left you feel it in the right).
I have tried all of these and found that my erect penis will point in the direction most stretched in. This can be undesirable . Stretching strait outward and slightly up has yielded the best results. Now my erect penis has a more natural and even look to it. Before extending I had an hour glass shape and a hook to the left.
Now:
Ligaments have the tendency to return to normal when you don’t work them out anymore. Basically, they will shrink to their normal size when you stop stretching (quit altogether). It is a good idea to perform some sort of hand stretching at least every other day to prevent losses. Consider it, as routine maintenance.
As you stretch the ligaments they will get bigger and stronger. As the ligaments get strong they will be able to resist stretching which will result in more force being applied directly to the tissue/chambers of the penis. The cells will divide (or get bigger. I’m not sure what the real science is) resulting in permanent penis enlargement.
From my experience:
1) I have not seen permanent results from using an extender within the first month.
2) I have seen permanent results after going for 2 months or more for at least 2-3 hours a day.
