From what I have heard some say you get more stretch of the ligaments with it faced down and more tunica faced up but as I mentioned in previous post there are extenders designed to be worn specifically in the upward position, so I do believe you get similar results regardless of the way it is facing. Similar to the PF having a vacuum extender tied around your waist seems it would work but I have not seen any studies indicating there success. Like the PF I would use the waist extender as a all day extender to just keep it in a elongated state, but your result will come from the use of rod extender. Since you already have the vacuum bell you should look at my post about how to modify your extender so you can wear it for longer periods of time.
Tem 4 meses que tô usando o sistema a vácuo, uso todo dia por 4~6h antes eu tava usando o de hastes, foram 2 meses mais eu demais, esse a vácuo traciona muito pouco vi comentários que se força demais rasga a camisa, fora as manchas vermelhas na Glande, aí quando eu uso a bomba fica pior ainda
