I may not understand the question. The hanger will slide down somewhat if enough weight is used. Rather, the internal tissues compress and make it look like it slides down. But the hanger itself should still be well behind the glans.

No, just start out with one stretched inch behind the glans. When weight is applied, it may look less.

Skin stretch and tunica stretch are two different things. The further toward the base the hanger is attached, the more skin stretch. The further toward the head, the more tunica stretch. However, the head is the last line of resistance to the weight. The entire inner shaft (tunica) will receive the stress as long as the skin does not interfere.

If you have a totally flexible schedule, with plenty of private time each day, then work until you reach total fatigue. Then, take a day or so off. If you have days where you cannot hang because of time limitations, then try to work you schedule around that off time. Hit it hard in the days before you know you will have to take off.
