Most guys do. As long as you are doing it properly. Not stretching only skin, changing angles as needed, etc.

Yes, eventually. It is that way with any type of PE. If you are applying the stresses needed to increase size, your penis will become stronger, and it will require more stress to make additional gains. That is one reason many guys plateau fairly early with stretching or jelqing. The greater stresses required are hard to generate.

When you no longer reach a fatigued state with the private time you have available, and at your current weight, and/or you no longer are making gains, you have several options:
You can increase the stresses by increasing weight.
You can try to increase time by use of an ADS device.
You can change the angles of stress, still basing your attack on your LOT. Things such as fulcrum hanging, or other unique angles.
You can take an extended break allowing the collagenous tissues to become weaker.
PE is a catch 22. As you gain, it is harder to gain because of the increasing stresses needed. This has been true for almost every one of the thousands of guys that have been in the various forums.
One of the most interesting theories I have ever seen discussed was the theory of getting the gains as quickly as possible before the tissues had the chance to increase significantly in strength. That weakened tissues should be easier to manipulate than stronger tissues. This is something I always had in mind when I was going for gains.
