Having just re-read Pezzi’s books, his thesis is that finasteride taken for a not less than six months drops DHT so much (say 70%) that the few androgen receptors that remain in the penile tissue of a mature male become sensitized to DHT. Pezzi is fair in stating that the biochemistry is not well understood and he suspects that there are, in my words, cofactors that work together with DHT to effect a burst of penile growth once cycling is stopped. Pezzi emphasizes NO ENLARGEMENT EXERCIZES whatsoever during the androgen deprivation phase because of the potential for severe regression - but, once off of finasteride for several days, then to resume PE activity. Ranges of finasteride mentioned were 1-5 mg daily and a more accurate doseage could only be fine tuned by tracking serum DHT via blood samples (my take).
What concerns me most about this apparent one-time event of Pezzi’s is that 1) ~80% of the gains were lost over time, and 2) I saw no evidence of any repeat trials. Still, in an attempt to be fair, if PE were not rigorously pursued after the cycling so as to solidify the gains, losses would result. As for caveat 2 above, 6 months of finasteride is not a trivial undertaking and I suspect few men would do all the testing necessary as well as take the finasteride in face of an uncertain outcome as we all vary enough in our individual biochemistry(s) to raise the spectre of failure on this.
In conclusion, I believe Pezzi is correct in asserting that the real challenge in enlargement is not that there is insufficient DHT - rather, the attenuation of the population of viable androgen receptors over time becomes accute and the growth/regrowth limiting factor (stem cells anyone?).
