For beginners, a moderate jelqing routine is best and recommended. Once your dick is conditioned to the new stress, there are better techniques for specifically targeting girth; like clamping, ULIs, and horse440s. They are all good exercises and I don’t think I could coin one as "the best". Some just seem to work better for different people. However, I would strongly recommend a jelqing only routine for awhile to avoid possible injury.

I’m not entirely sure what you mean by "type of jelqing". Most jelqing is pretty much the same; designed to force large amounts of blood through the penis, increasing the internal pressure and creating controlled damage or micro-tears in the structure of the penis. One variation that I like is BTB (behind the balls) jelqing. This really helps prevent the "baseball bat" look.

Yes, I do. The difference holding back the base skin makes for me is two fold: 1) It controls skin stretch. I’m really big on keeping the fat pad where it’s supposed to be, at the base of your dick, not riding up your shaft. I’d rather have just my dick be stretched out. Holding back the base skin while I jelq helps me achieve this. 2) I can get a better, deeper grip when I push back on my fat pad at the base. The lower (closer to your body) you can start your stroke, the more cumulative effect it will have on your entire dick.
