Though I’m not sure about the physiological factors that may play a role in this, I have heard of some people who are just starting out that have gained just from doing Kegels (which help improve blood flow/erection strength/etc.). I would think that the reason behind some people gaining from kegels would be due to there being more blood in the unit, which would add more pressure on the ligs, thus causing them to further expand/stretch.
An example of this could be:
You have an empty balloon.
You start to fill the balloon up with water.
As you continue to fill the balloon up with water, the more water you put in it, the more the ‘skin’ of the balloon stretches.
The water would be the blood in your unit.
The ‘skin’ of the balloon would be the ligs/tunica of your unit.
As you can see, the additional pressure placed upon the ‘skin’ of the balloon via water causes the ‘skin’ of the balloon (ligs/tunica) to expand/stretch further.
This, however, doesn’t mean that you can gain indefinitely just by increasing the blood flow to your unit - your unit itself has to actually be bigger to have the potential to hold more blood in it. You can’t just keep putting water in the balloon, eventually the balloon will have no more capacity to hold any more water. If you wanted it to hold more water, you’d need a bigger balloon.
