Just as with regular jelqing, it’s almost impossible to do a BTB jelq without 20% erection at the very least. It’s especially difficult finding the penis root precisely because it isn’t “out there.” With a good, solid erection (like a morning woody), it’s fairly easy to feel the penis—starting at the base, and then back through the top of the ball sack (scrotum) and into the taint (perineum).

Just as with regular jelqing, there are optimal levels depending on how experienced you are with jelqing. The vets here would not recommend doing jelqs at higher levels of erection until you’ve been doing PE for a while precisely because you can overdo jelqing. That’s why, as a newbie, I only did some gentle jelqing with a 100% morning wood. With less of an erection, I can do some jelqing of the shaft hidden within my ball sack, but it’s not as easy to feel for the shaft then.

As with regular jelqing, sore is OK; pain is not. One difference with BTB jelqing is that you can also be putting pressure on other parts of your anatomy—like the prostate. It’s almost impossible not to put some pressure on the prostate and maybe on the Cowper’s gland with BTB jelqs. That’s why you have to have enough erection to feel for the “hidden penis” and know what you’re actually “milking.” Look up one of the detailed diagrams on TP and you’ll see what I mean. If your prostate is sore after doing BTB jelqs, that’s not what your aiming to do with this. As far as a “pumped feeling,” BTB jelqing wouldn’t be much different than other forms of jelqing. At least that’s my own experience.

Not any more than regular jelqing can replace kegels. Both will help push blood further up CC, but jelqing works on the tunica (which you cannot “flex”), kegels work on voluntary muscle tissue (which you can literally “flex”).
I’m pretty new at all this, so I’m hoping others who have done a lot more with BTB will add their comments.
