1) If you have made girth gains, have you noticed considerable gains in the CS?
Yes, as well as in the corpora cavernosa.
2) What girth exercises have you used and gained most from?
I’d say Sadsak Slinkies combined with clamp exercises and Ulis. I try to to soothe the tunica, to render it more malleable, and then to expand the corpora cavernosa by squeezing with exercises like Ulis.
2) Do you make a point of kegeling while working on girth? Do you think this may have been associated with gains?
I don’t think regular kegeling and girth are directly related, but it’s better to have a well-toned PC muscle to kegel when you do girth exercises.
3) Is most of your girth work done while fully erect?
No; perhaps the half. My favourite routine for girth is:
-warm up (with hot water)
-5 min dry jelq.
-5 min jelq squeeze.
-5 min Uli (Uli#3) and Sadsak’s head exercise
-4x15 Sadsak Slinkies (15 in each direction), slowly (3 seconds go, 3 seconds return)
-15 Horse squeezes of at least 30 sec. each, with very little rest between them.
-15 min. clamping:
* 5 min. standard, normal clamping
* 5 min. clamped "over the counter"

* 5 min. extreme clamping (i.e., an Uli just squeezing the glans; I usually kegel constantly)
-some Uli (Uli#3) and Sadsak’s head exercise.
I usually finish wearing a not very tight cock ring for a couple of hours.

Yes.
5) Has your CS grown uniformly? That is to say, is the base area of your CS thicker than the rest?
Yes, the base of my CS is clearly thicker. Nevertheless, I noticed nice and quick results in the rest of the CS with this almost forgotten exercise, the CS blasters:
