What is the longest amount of time to hang at 1 session? Is 20 minutes optimum?
What if i hang heavier for a shorter amount of time, like 20 lbs for 10 minutes?

I’m asking because i was hanging SD, SO over the desk 10lbs at (3) 20 minute sessions for a month, and nothing changed
I should add weight correct, so i went SD, SO over the desk at 15lbs (2) 20 minute sessions with fatigue setting in faster for a month and nothing changed, thinking about adding that 3rd session

Big question is, is it better to hang lighter weights for longer time, or heavier weights for shorter time?
If i stick with 10-12lbs, i can easily go 30 minutes

Also placement of the hanger. I do place 1 1/2 or so behind the head, but what about towards the base?

20 minutes, because any longer & you might create tissue death (this is how long tissue can survive without oxygen).

The more weight you use, the higher the chance of causing injury. Doing less weight//more time is always safer. Of course this must be within reason—you need enough weight to reach fatigue if you expect gains (usually by your second 20 minute set). Avoid high weight/ short sets, unless you want a macro tear & permanent injury (check out some of the video links on this forum—we’ve got chinese guys hanging 400lbs for a few seconds from small dicks—that should tell you a lot…)

Are you hanging daily? If not, there’s your problem right there. Consistency is critical for gains, you need constant deformation to slowly create growth & regeneration resulting in permanent gains. If you stop, your dick can actually heal shorter.

As stated already, you need to reach fatigue. Amusing you’re getting good fatigue, why use more weight than needed to do this? you’ll just be creating an injury risk.

Always hang 1” to 1.5” behind the head (right behind the circumcision scar if you’re cut). Sure, you can hang right from the base if you want—but this will result in extreme skin stretching. You can try to be like Bib & create your a new foreskin. If you’re already cut & you do enough hanging from the base, eventually you’ll have enough skin to cover your glans while you’re flaccid.
