I think it’s the method; FWIW I certainly wouldn’t call it my method. I’m sure others do it too. If it’s not described on his BH site, it should be.

Have you ever thought about a hanger that was hinged at the top and tightened with wing nuts down the bottom?

Only briefly, and it’s not really on the backburner either. I should say I don’t put much thought into hangers now; I will probably never need another beside my well and truly modded beta BH. There’s aldo enough substance on this forum - maybe even in this thread - for anyone to go right about making their own optimal hanger mod.
A top hinge then, would surely be a one point joint such as a ball joint or even a simple [semi-]permanent tie, since a multiple joint hinge will necessitate some elaborate workarounds to enable toe in/out settings, defeating any purpose of simplicity.
I don’t see a top hinge being quite essential to an ultimate design clamp style hanger. Looking for conquests, I would first pay attention to the reverse side of the hanger.
You also asked me about the basal teeth grinding, and I hope you don’t mind me replying to that here.
The reason I don’t want the basal teeth overlapping the center line - not by much anyway - is that it will allow little channelling, resulting in fluid build up in the frenulum area.
