You should hang with the most intensity for the most time that is confortable for you. I know, vague. Let me explain. There are people who have the privacy/time/whatever to hang hours and hours per day. There are other people who are only able to hang one, two hours at most (and probably not all days). Whatever the time you have, you should hang to the point of fatigue IMHO. But not now, when you just begun. You have to accomodate to those stresses, gently and patiently. So at first start with 3 sets of 20 minutes with 10 minutes rest in between. Light weights. Get the hang of it (so to speak). Then when you feel that you’ve got all the variables pinned down (hanger position, wrap, tightness, etc) you can start increasing weight, by small increments. For instance, when I started with the big hanger, I started for 3 sets of 20 minutes with 2Kg (~4.4 lbs), 2-3 times per week. Now I’m hanging 6 days per week, 5-6 sets of 20 minutes with 10Kg (22 lbs). I add weight about every two weeks or when I feel that the weight I’m using is not stressing the ligs/tunica anymore.

IMHO you shoud concentrate first on one thing, length or girth. That way your focus is less dispersed and you’ll be able to make better gains. As you probably know jelqing builds mainly girth, hanging builds length. There’s nothing wrong with doing one after the other. It could even be beneficial to stretch lateraly the tissues lengthened by the hanging, to do a couple of light jelqs. But it’s IMHO that you shouldn’t do a full-steam routine of jelqs (or other girth exercises) after hanging, or vice-versa.

I noticed today a new article explaining the LOT theory. You should have a look at it. As for the hanguer position, whatever works for you. When first starting you should experiment with light weights, the different hanger positions, to see which is more confortable to you.
