Discomfort and as a consequence not hanging long enough or using enough weight.
I used to wrap over the head to prevent twisting. This worked but wasn’t as comfortable as traditional wrapping.
Wrapping to the head - as in starting the wrap right behind it - caused problems. No matter how hard I tried to make sure all the skin was covered, often a tiny sliver was left bare, usually under the head. Bad situation. That bit of skin gets pinched between the front of the wrap and the glans, resulting in a nasty, painful, blister-like fluid buildup after only a few sets. Even when I managed to cover all the skin, the front edge of the wrap was in direct contact with the head. Again, uncomfortable.
Starting the wrap an inch or so behind the head works best for me. This leaves enough free skin in front. When the hanger moves slightly forward the skin can roll partially over the glans.

Could be. You should have plenty of skin available at SO. Try wrapping farther back and see how it works out.
