You raise a number of issues here
A tpr ring at the base of the cylinder is effective to avoid testicles from entering the cylinder. Another option I have used is stainless steel scrotum rings which make your testicles hang a bit lower. My Mr Fantastic water pump cylinder uses a tpr ring at the base to seal the water in so I am actualling using that and the scrotum rings. You can certainly use a silicone pumping sleeve while doing this.

Do not think it is a good idea and don’t think it would prevent ball suckin.

The general view is that it is a good idea to pump erect. Probably easier to fit the sleeve while you are erect.
