In theory it should stay constant. In real life there are several things which can cause a decrease in the vac level.
As your penis increases in size, the vac level will decrease. Once dick size has stabilized, the vac level should also. If you kegel while in the tube you can see the gauge needle move up and down slightly as your penis reacts to the kegeling.
Sometimes I pump to a higher vac level (7-8inHg) to ensure a tight seal, then back down to 5. I also twist the cylinder back and forth a little into my pelvic area for the same reason.
Pubic hair is one of the greatest source of poor seal.
If you are moving around you can cause micro leaks. If you need to move, press the tube in and hold it until you re settle.
Support the tube with a pillow (or whatever) to keep it in an upright position and minimize it flopping around.
Other than these, you’ll need to check fittings and connections for leaks. Check that the pump release valve is fully closed. Some types have a tendency to stick open a little. Also pay attention where you place the pump to make sure the release valve is not making contact with something.
