Lots of guys come here seeking injury advice.
Often they say they have, for example, hard flaccid, or can’t get a proper erection after doing some PE. Then they ask what’s wrong and what to do about it.

Doing that may get you some advice, but you could get much better advice if you give more details.

Only a medical professional can give a diagnosis, and for that they will need to see you.
The best you can hope for here is some good advice from someone who has been through what you are experiencing, and have found what worked for them, or who have other knowledge of your condition. There are doctors here, but they are not going to advertise that fact, nor would a genuine medical professional of any type ever try to diagnose your condition over the web.

So what questions should you answer to get the best advice?
Here are some examples - you would do best to answer them all, but you could pick and choose from the list. Copying and pasting them into your post is a good idea, along with your answers.

Age?
General health (are you fit enough to walk a mile fast, or play a full game of basketball etc. or are you more of a couch surfer, for example)?
Do you take regular exercise? Aerobic or resistance or both?
Under/over/normal weight for height?
Do you smoke? How much?
Do you use other recreational drugs (including alcohol) Much or little/occasional or regular user?

Are you taking any prescribed medication? (Include anything which would be prescribed but that you get elsewhere, e.g. prescription painkillers, ED meds etc.)
Are you taking any over-the-counter medication? (e.g. antihistamines, NSAIDS etc.)
Are you using any dietary supplements?
Do you generally eat a healthy diet, or do you live on fast food and cookies?

Do you suffer from anxiety, stress, fatigue, depression etc.?
Are you typically physically relaxed ot tense?
Do you have any known health conditions (e.g. cardiovascular disease, diabetes, neurological problems, hormonal imbalances, thyroid disorders, asthma etc.)?

What symptoms do you have?
How long have you had them?
Have they been the same over that time or have they got better/worse?
Have they improved for periods of time (e.g. when you are on holiday or doing different work)?
If symptoms have improved or gone away for periods of time, how long did they improve for?
What makes the symptoms better? (think hard about these - anything from less PE/masturbation to drinking more H2O to doing meditation etc. is useful info)
What makes the symptoms worse? (think hard about these - anything from a hard day at work to alcohol or lack of sleep etc. is useful info)
Are there certain times of day the symptoms are better or worse?
Are the symptoms better or worse when you are in certain positions (e.g. sitting, standing, laying prone or supine)?

Have you seen a medical professional with your symptoms?
What type of doctor?
What did they say?
Did you have any tests?

How do you think your condition/symptoms occurred? (Be very honest here, or no-one can really help you, e.g. don’t say you did a few gentle jelqs if you strangled you penis, or that you did a bit of stretching if you stretched as hard as you could, or that you tried clamping if you wore a constricting device all night…)

Is there anything else you can add that may be useful to know about yourself or your condition?

Penis Injury

I’m 18 years old I’m considerably fit. I’ve been doing jelqing for about two weeks, I started to see results and became excited and wanted to see results faster. After 2 weeks 20 reps of jelqing was not doing much, so I decided to search a different exercise, It’s where you stretch your flaccid penis in all directions (I did it pretty hard, I didn’t feel much pain so I thought it was okay), but immediately after finishing the exercise I knew something was wrong, my flaccid became smaller instead of becoming larger, and it felt very uncomfortable.

At the time I didn’t know about penis injuries, so I kept doing the same exercise and a switch to jelqing for about 2 weeks. Now my penis feels swollen with blood and looks smaller when I’m hard and soft. Mine used to stand at 6’7 inches and now decreased to 6 inches. I decided to search what’s happening to me and found out I’ve might’ve over trained. I’m confident that this is an injury, but not if my penis becoming smaller is part of the injury. I can be rock hard and stand at just 6, I remember clearly my penis being bigger than that 2 months ago. I’ve rested my penis for the past four days but ejaculated today. Can my penis recover by itself or should I go to the doctor and plan surgery? Will my penis ever go back to its size? Should I ejaculate during recovery? And how long will it take for me to recover? I want to get back into PE and reach my goal of 7’6.

I’ve often had ‘shrinkage’ in the past from over-doing stretching, clamping or pumping. It’s just the tissues protective reaction. A couple of weeks of leaving your penis alone (very gentle masturbation only if absolutely necessary) is almost always enough to sort it out. If you’ve done quite a bit of PE while the tissues are trying to protect themselves it may take a bit longer but it’s unlikely you’ve done any major damage.

Strained ligament

Hello Firegoat, first off I think this is a great thread. I am pretty new to PE, got hooked on it in July. I started pretty cautiously, and am pretty aware when I am over doing it. About two months ago, I began to see some real growth, and started to increase manual stretching. (Typical excitement which leads to an injury).
One day when I was pulling BTC, I felt a sting at the base of the right side of my penis. I was alarmed, but I might have finished the session, trying to avoid pulling the injured side. Unsure of what I did, I took a short break before freaking myself out online. However, the slight pain did not go away. It kind of felt like a bruising, and would increase in pain when erect. I did some research and was pretty sure I had sustained a ligament strain. Online it said 3-6 weeks of doing nothing, and I was really bummed out. Over the next couple of weeks, the bruising pain would go up into my abdomen area. I took ibuprofen, and iced occasionally. After about two and half weeks, the bruising pain started to die down. So I decided to try PE again. There was not really any pain during, but after it felt bruised again, and I felt a slight pain while erect. (on my break I was not masturbating either) I realized I was not fully healed, and took another two weeks off. I would feel barely anything, maybe slight discomfort where I sustained the injury, during the break. After two weeks, I tried PE again - with the same result. I thought I was fully healed, but I would feel slight pain while erect, and a bruising pain in the area afterwards. I took another week off, this time thinking I was fully healed tried again - yet I felt the bruising pain, and erect pain again.

I know I just need more time. However, unsure how much longer. I am also losing my size when erect and flaccid which is annoying. I am wondering if you can give me some advise, or warnings in case you have had this injury before. Also wondering if gained size will come back easily when I start again, and advise in how I should start my routine again.

My routine before being injured was 2on -1 off with manual stretching. And 1on-1off with jelqing, with edging afterwards. Focusing more on stretching with 10 min sessions in hot shower.

I am 21, going to college. I work out maybe twice a week, used to be big swimmer, so still fit from those days. Meditate twice a day, and currently do not smoke. Thank you

Hi kong. Yes, it sounds like a ligament strain. Just because the pain has subsided does not mean it has fully healed. Ligament strains usually take a minimum of 6 weeks to heal, and you still shouldn’t push them hard for a while after that. Start very slowly and gently and back off immediately if there is any dicomfort at all.

Masturbation, as long as it isn’t too aggressive, shouldn’t be a problem - just avoid stretching the ligament while it heals. If you have irritated the injury a few times you risk getting into a state of chronic (ongoing) injury where tissue memory will cause an over-reaction of inflammation in response to less trauma than was previously needed to trigger it. That state can be broken by avoiding any aggravation of the injury, but it often takes a more extended period of time of totally avoiding inflammation, often 12 weeks or so (it is never worth trying to train with a ligament injury, whether it’s a penile suspensory ligament or any other ligament in your body). Often you need to ice at least 4 times a day (over a tea towel or t shirt of course) to help break the inflammation cycle. Very, very gentle stretching after the 4-6 week period can help, but it needs to be very slow and gentle.

Hopefully you haven’t got into a chronic inflammation state, and the injury was fairly minor to start with; it sounds that way from what you are saying. In which case a proper 6 week break from any PE (gentle masturbation is fine), followed by easing into it very slowly and gently over a few more weeks will sort it out fully. If you get any discomfort after any activity, ice it for 10-15 minutes (depending on how cold your ice pack is).

Any lost size will come back much faster than it took to get in the first place. Lost size is usually just elastin in the collagen matrix returning to pre-PE state - it wasn’t ‘real’ gains to start with. But tissue memory will return the size quickly. It’ll take a lot longer before extra size becomes permanent anyway.

Thank you for replying, that was very helpful. So I should rest another six weeks to fully get rid of any lingering damage, and to not induce the re-occurring inflammation? Also not do any very light stretching until after that time, or some during?

Yes. If there is no pain you can do a minute or so of light stretching. It doesn’t sound a bad enough injury to have developed any ‘real’ scar tissue (no visible bleeding/bruising), but any ligament sprain will have caused damage to a few fibres so think of gentle stretching more as gently helping to re-align/re-educate the tissue than as ‘PE’. If you get any discomfort during the light stretches, stop. If the area feels any slight soreness after stretching, ice it and give it a day or two off before trying again.

I’m a 33 year old in good health and I resistance train 4 times a week. I have long term depression and the only medication I am taking is liquid Cialis. I injured my unit a year ago from pumping at very high pressures. Right now my boners are pretty weak and I believe the skin of my shaft has separated from my penis causing it to look dark and wrinkled. I am hoping to recover still but was wondering if it would be beneficial to start pumping again but at a resonable pressure. Also would you know if tolerance builds while taking Cialis and if so does it platue at some level? Sorry if I’m being to vague. I not exactly looking for a diagnosis, I’m looking for advice on helping blood supply to my unit.

Hi SK.
It is physiologically impossible for the skin of your shaft to separate from your penis. I’m assuming the Cialis you are taking is not prescribed but self administered and dosed. Long term depression is a prime cause of weak boners, and I suspect that is the case with you as any injury you sustained a year ago would be fully healed by now.
If I were you I would try some short, low vacuum pumping to help re-train your penis how it feels to feel fully erect (and to help get oxygen and nutrients into the tissues). See how it goes. Continue if it’s positive and reduce or stop if not.
You do not build tolerance to Cialis. If you are taking it every day it takes around 3 days to reach a level in your system and if you keep taking the same dose it will remain at that level. The effectiveness remains the same, although subject to normal fluctuations in hormones, mood etc. just as a non-chemically enhanced erection is variable.

Thanks for the quick reply. Yes I have been unemployed for about a year as well so I think you might be right about the depression. Maybe the extra time on my hands has made masterbation a easy escape and I have relied on it to much for comfort and dopamine. I also quit meth about a year ago (after a 2 year binge) I don’t know if this is part of the problem as well. I will definitely take your advice and tread carefully with pumping. I used to over due it quite a bit at 3 times the sujested top end, maybe that’s 3k pressure? I always thought the excess dark wrinkled skin I acquired was from the skin pulling away from the penis but I’ll take your word on that it is not.
