I would start with the newbie routine and go from there. I’ve never graduated from it and have had decent gains (you can check out my log). I too am shooting for 7 inches. We are starting at comparable lengths so even if you made the same progress that I have you’d be at 6.5”.
To your other two questions. Should you get surgery? I would say no. I’ve looked into some phalloplasty surgeries for turkey neck and that makes me nervous. The other ones scare me even more. The ligament surgery really only buys you as much as you can get from stretching anyway (at least for me it has). The other ones are surrounded at worse with horror stories and at best with results that I wouldn’t want for my own dick. I’m really big on the look of a penis over the size. While a lot of guys don’t mind, or maybe even like, that their penis looks morphed I certainly do. It’s like the people that enjoy looking obviously like they have plastic surgery because some other feature is “perfected.” I am not one of them.
Lastly, no, ten inches is not possible for you. You are starting just south of six inches. You’d have to pick up over four inches in length. That would be beyond a record. More realistically you could get to the 6.5-7” range. You have good girth and will probably pick up even more along the way so that will be a bonus too.
