Persistent muscle tension is a known symptom of anxiety. Anxiety has also been known to cause cold hands/feet due to lack of blood flow to the extremities, including the penis.
To prove further that it is caused by a lack of blood flow, think about how the penis normally shrinks into a firm flaccid states when exposed to cold.
As far as treatment goes, think about the usual non-medicated treatment of depersonalization/derealization. Both of those disorders are caused by high levels of anxiety. They are persistent disorders. These are usually cured when the person suffering learns to surrender to their disorder, and eventually stop worrying about it, and by it’s own nature it will disappear.

To conclude, it seems as though firm-flaccid can be seen as persistent performance anxiety, which can be overcome by removing all anxiety and tension surrounding the issue and becoming relaxed. Do not think about the issue, or ever remind yourself of it, and it will disappear. Do not even take note of state of the state of your penises while pissing, this wouldn’t be considered normal had you not heard of this issue, and is a sign of anxiety. If you still have the urge to check your penis, you still have the anxiety.
Nope, that’s a completely different sensation, proof that you yourself don’t actually understand the term "firm flaccid".
My penis used to retract and go smaller when it was cold, it did not, however, go hard like an erection in the flaccid state like it is now.
