I have read about people feeling fatigue there. I believe they called that area the “gooch”. I haven’t really had fatigue in that area.
However, I believe that what you are doing is far closer to extenders than what most traditional hangers do.
If you get a chance you might want to check out the extender part of the forum and see if some of their topics applies to you.
The fatigue I feel is usually at the base of my penis and around that area. However, this could be because of the type of hanger that I use, the weight, and the angle that I hang.
