Actually, another member private messaged me regarding the setup. I’m going to copy/paste what I wrote to him here:
I agree, devices are difficult to describe, however I will do my best:
1. The web belt is a typical strap you’d use to tie up a sleeping bag or something. It consists of the strap, an end with a slide attached. It also has a hook, and is set up so that the hook is in a loop where the slide is able to change the length of the strap. The strap is like this:

2. The raw end of the web belt is modified for traction on the penis. I used a knife to shred it up a little bit, and then coated the end in silicone. I used Permatex gold, but I imagine any heavy duty sealant would work. The side that you will place against the penis should rest on wax paper or something that will leave a smooth finish. The strap should have about 1.5" of non-coated strap, then about 2" long of coated strap, then the remainer of the strap uncoated.
So, now you have the complete strap. The strap can go around the waist, or as I’ve found, shortened up and put around the leg so you can do an inverted V stretch.
Then you use a large O ring, or I am using a cock ring, made of nitrile rubber. Large diameter, because you’re using the stretchiness of the ring as your tensioner on the unit. I use this ring in conjunction with a polypropelyne sock rolled up to provide a cushion.
Then the more difficult part to master, attachment to the head:
I use latex finger cots (I am NOT allergic to latex, if you are you have to find some other kind of sleeve, silicone or something, but they are not as readily available)
In a completely flaccid state, I get the glans into the cot, then pulling tight, roll the cot down the shaft. It is very important that you do not get any skin folding inside of the cot, it hurts, believe me. Also, it is important to get the cot tight around the glans, if there is any loose on the end, kind of like a reservoir tip of a condom, then it’s going to hurt when you take it off.
Holding the cot tight, place the strap against the top of the penis so that the silicone is right on top of the glans ridge, and just behind the glans on the shaft. Using 1" wide self-adhering wrap, about 6" long, wrap the strap to the penis. I find that if I place the wrap where it just barely covers the ridge, and holds tight against the shaft just behind the glans is the best place. You might be different.
Now, once the 1" wrap is secure, use 3" wide wrap, about 8" length. Use the 3" wide wrap to wrap over the entire glans, and down to behind where you have the 1" tape. Make one or two turns, then fold the length of strap that is sticking out behind the wrap to double back on itself. Make the last turn or two of 3" wrap.
Lastly, and this is important too, Pull the ring of the cot to double back onto the wrap so that it is not touching the skin. This holds the wrap in a good place, but also keeps that ring from digging into the shaft while stretching.
You should be able to get a good tug. YOU ARE GOING TO HAVE TO TWEAK THIS.. Quite a bit, to understand exactly how it works.
