Let me see if I can explain this for you and make a crude drawing. I believe this is a fulcrum stretch using your fingers and the hand your grip with to tension head of your penis
opposite of the side with the fulcrum. Or in other words, it is a A or V Stretch where you “pull” the head of your penis into the direction of the finger used to create the fulcrum point.
So what you would do is take your penis and with your left hand grip near the head as if you were to do a manual stretch, and stretch your penis in the 9 O’clock direction. Now with your right hand, place your finger(s) on the top side of your penis at approximately mid-shaft and press downwards. This should cause your penis to form a approximately 90* bend around your finger and your left hand should now be raised higher than your right hand with the top half of your shaft being vertical and the bottom half being horizontal. Now to create the second fulcrum or pivot point for the shaft of your penis, you will take your left hand (the one gripping hear the head),and you will want to rotate your wrist downwards(in the direction of your fingers creating the first fulcrum.) This should cause a second fulcrum near the grip on your penis, thus giving you two.
This is a crude model of the stretch, with points A being location of a fulcrum created by your finger(s) as in a V Stretch, and location be being the point of your second fulcrum created by the hand you are gripping with pulling back towards point A. The periods are there to keep the shape of the model, with the lines being your penis.

Or imagine you have a rope tied to the wall and two metal rods fixated in place which are point A and point B. You want to pull it straight out and under point A(your finger). Then you take the end you are holding and stretch it all the way up to point B(your grip hand) and then back down over point B towards point A.
